Terapi Murottal sebagai Pendekatan Non-Farmakologis dalam Menjaga Stabilitas Hemodinamik Pasien ICH di ICU RSUD dr. Tjitrowardojo Purworejo

Perdarahan Intraserebral (ICH) merupakan salah satu bentuk stroke dengan tingkat morbiditas dan mortalitas yang tinggi, sering kali disertai dengan ketidakstabilan hemodinamik terutama pada pasien yang menjalani perawatan intensif di ruang ICU. Pendekatan non-farmakologis seperti terapi murottal Al-Qur’an berpotensi memberikan kontribusi terhadap stabilisasi kondisi fisiologis pasien. Tujuan penelitian yaitu mengeksplorasi pengaruh terhadap stabilitas hemodinamik pasien ICH di ICU RSUD dr. Tjitrowardojo Purworejo. Penelitian ini menggunakan desain studi kasus deskriptif yang melibatkan 2 pasien dengan diagnosis ICH dan masalah keperawatan Risiko Perfusi Serebral Tidak Efektif. Intervensi yang diberikan berupa pemutaran murottal surah Ar-Rahman selama 15 menit per hari selama 3 hari berturut-turut. Parameter hemodinamik yang diamati mencakup tekanan darah, denyut jantung, laju pernapasan, suhu tubuh, saturasi oksigen, serta skor Glasgow Coma Scale (GCS). Hasil studi menunjukkan setelah intervensi, kedua pasien menunjukkan perbaikan parameter hemodinamik. Selain itu, skor GCS pasien mengalami peningkatan atau stabilisasi tanpa indikasi penurunan tingkat kesadaran. Bisa disimpulkan bahwa terapi murottal Al-Qur’an surat Ar-Rahman terbukti memberikan efek positif terhadap stabilitas hemodinamik pasien ICH di ICU. Pendekatan ini berpotensi mendukung pemulihan pasien kritis secara holistik dan integratif.

Pengaruh Penggunaan Ankle Foot Orthosis terhadap Kualitas Cara Berjalan pada Pasien Stroke

Stroke is the main cause of motor disability, one of which is walking disorders due to hemiparesis. This study aims to determine the effect of using Ankle Foot Orthosis (AFO) on walking quality in non-hemorrhagic stroke patients. The method used is a case study on a 58-year-old female patient with left hemiparesis. The intervention in the form of using AFO was carried out for five consecutive days, once a day, with daily evaluation of walking quality using observation sheets and recording walking time of 5 meters. The results showed an improvement in walking after using AFO. On the first day before use, the patient walked draggingly and needed 27 seconds to cover a distance of 5 meters. After using AFO, there was an improvement in posture, stability, and lifting of the left leg when stepping. Walking time also continued to improve during the five days of intervention. In conclusion, the use of AFO in stroke patients in the acute phase can help improve walking quality significantly. This intervention is effective as part of early mobilization efforts to prevent further decline in function and increase patient independence.

Academic stress among nursing students is related to the demands of meeting competency achievements from both coursework and field practice. In reality, students’ abilities to fulfill these competencies vary. However, due to these demands, students are required to perform both academic activities and clinical practice. This study employs a descriptive quantitative research method with 60 respondents. The sampling method used is Accidental Sampling. The instrument used in this study is the Educational Stress Scale for Adolescents (ESSA) questionnaire, which consists of 16 statements related to academic activities, measured using a Likert scale. The results show that the majority of nursing students (75.5%) experience moderate academic stress. In this study, the moderate stress category among students is associated with their adaptation process to a new learning system and the demands of achieving both academic and practical competencies.

Efek ERAS (Enhanced Recovery After Surgery) pada Pasien ORIF dengan Spinal Anastesi: Studi Kasus

This study aimed to evaluate the effects of the Enhanced Recovery After Surgery (ERAS) protocol in a Post-Open Reduction Internal Fixation (ORIF) patients with lower extremity fractures. The case study was conducted on two patients: an adult patient with a closed-tibialis dextral fracture and an elderly patient with an intertrochanteric femur sinistral fracture. Data collection methods included direct observation, interviews, and the utilization of ERAS-standardized assessment tools such as the Visual Analog Scale (VAS), Hamilton Anxiety Rating Scale (HARS), and early mobilization observation forms. The results showed a decrease in pain within the first 24 hours post-ORIF from VAS 5 to 2, a progression of early mobilization from sitting to standing within a day, and an anxiety reduction. The discussion emphasizes the key of ERAS components: multimodal pain management, early ambulation, and patient education. The study concluded that ERAS is effective in enhancing both physical and psychological recovery in post-ORIF patients especially to manage pain, improve mobilization and reduce anxiety.

This study develops an android-based broiler chicken cultivation guide application that provides information on how to cultivate broiler chickens. Although this cultivation effort can be done by anyone without special skills, it still requires guidance so that the business can run smoothly and profitably. Information on broiler chicken cultivation guides is widely available in various sources, such as books, online articles, and video tutorials. However, the information shared usually only discusses broiler chicken cultivation in general terms, which may not be in accordance with the needs and conditions of each farmer, making it difficult for farmers to find complete and efficient information. An Android-based broiler chicken cultivation guide application can be a solution to overcome these problems. Application development is carried out using Android Studio with the Java programming language. In this application, blackbox testing is also carried out to measure the functionality of the application and whitebox testing to measure the correctness of the program logic of the application. This application has various features such as getting to know broiler chickens, broiler chicken cultivation preparation, broiler chicken feed composition, initial growth phase maintenance, finisher period maintenance, broiler chicken diseases, harvest and post-harvest, prospects and how to calculate farmer profits and losses, as well as broiler chicken cultivation videos that can help farmers in carrying out broiler chicken cultivation efforts.

Patients with chronic kidney disease (CKD) undergoing hemodialysis often experience sleep disturbances as one of their complaints, which can significantly reduce their quality of life. The aim of this study was to evaluate the effectiveness of a complementary therapy combining the recitation (murottal) of Surah Ar-Rahman with lavender aromatherapy in improving the sleep quality of hemodialysis patients. The study was conducted on a 60-year-old CKD patient in the inpatient ward of PKU Muhammadiyah Gamping Hospital over five consecutive days. The intervention was administered each night for 10–20 minutes before bedtime. Sleep quality was measured using the Pittsburgh Sleep Quality Index (PSQI). The results showed a significant decrease in the PSQI score, from a score of 10 indicating poor sleep quality to a score of 1 indicating good sleep quality on the fifth day. This study demonstrates that the combination of murottal and lavender aromatherapy is effective in improving sleep quality in CKD patients undergoing hemodialysis, making it a safe and beneficial non-pharmacological intervention in nursing practice.

Hypertension is a disease that occurs and is characterized by an increase in uncontrolled blood pressure. This uncontrolled disease can result in progressive disease that needs to be treated properly and continuously. Hypertension treatment can be done using antihypertensive drugs. One of the determinants of successful therapy is compliance with the use of antihypertensive drugs by patients. This study aims to provide an overview of the compliance of hypertensive patients in consuming antihypertensive drugs at the Outpatient Installation of Gunungsari Health Center. The design of this study was prospective and analyzed using a descriptive method. This study was conducted on hypertensive patients at the Outpatient Installation of Gunungsari Health Center who underwent control and received antihypertensive therapy where samples were taken using a purposive sampling technique. The data obtained will be analyzed based on the compliance category with the MARS-5 questionnaire. Data will be presented with percentages and pie charts using Microsoft Excel. The results showed that the level of compliance in taking medication for hypertensive patients at Gunungsari Health Center showed a moderate level of 71.58%; a high level of compliance of 18.95%; and low compliance of 9.47%. Factors that affect compliance are forgetfulness, laziness, difficulty remembering medication schedules, and not feeling symptoms of illness. Intensive patient and family education, implementation of reminder strategies, medical personnel monitoring, and innovation in technology-based education methods are required.

Gout arthritis is a form of joint inflammation caused by purine metabolism disorders, leading to elevated uric acid levels in the bloodstream (hyperuricemia). The accumulation of monosodium urate crystals in joint tissues due to this condition triggers acute inflammation and severe pain. Elderly individuals are particularly susceptible due to reduced metabolic function and decreased efficiency in uric acid excretion. This study aims to evaluate the nursing care provided to patients with gout arthritis, with a focus on pain management, sleep disturbances, and patient education regarding the disease and its management. This research employs a descriptive case study approach, emphasizing scheduled activity interventions for gout arthritis patients. Data collection was conducted through interviews, physical examinations, and observations, with findings presented in a descriptive case report format. The assessment revealed that the patient experienced severe pain in the metatarsophalangeal joint, with moderate to high pain intensity. Additionally, the patient struggled with sleep disturbances caused by pain and lacked awareness of the appropriate dietary measures for managing uric acid levels. Nursing interventions included pain management strategies, patient education on a low-purine diet, and supportive measures to enhance sleep quality. Effective nursing interventions for gout arthritis patients can significantly improve their quality of life by alleviating pain, enhancing sleep patterns, and increasing disease management awareness. Continuous patient education and adherence to therapy are essential in minimizing recurrence risks and preventing future complications.

The increase in life expectancy among the elderly is, on one hand, a positive development, but on the other hand, it can lead to new social challenges, particularly concerning elderly care and welfare. Changes experienced by the elderly, such as reduced income due to retirement, physical changes, and communication difficulties, also impact their quality of life. The quality of life for the elderly is not solely measured by general satisfaction but encompasses various dimensions, including physical, psychological, social, and environmental factors. From an Islamic perspective, caring for and showing attention to the elderly is highly encouraged, with an emphasis on the importance of emotional and social support. This study aims to analyze the relationship between family coping strategies and the quality of life of elderly individuals with mild cognitive impairment in the working area of the Banjar Serasan Health Center, East Pontianak. The research employed a descriptive correlative design with a cross-sectional approach. The study involved 50 elderly participants aged 60 years and above, selected through screening using the MMSE (Mini-Mental State Examination) instrument. The findings of the study concluded that family coping strategies play a crucial role in enhancing the quality of life for elderly individuals with mild cognitive impairment. Statistical analysis revealed a significant value (p = 0.000), result indicating a relationship between family coping strategies and the quality of life of the elderly with mild cognitive impairment. The correlation coefficient value of 0.663 suggests a strong relationship.

Acute stroke is a condition in which a patient experiences a disorder in the blood vessels of the brain that disrupts the flow of blood to the brain, and the disorder can be in the form of blockage or rupture of blood vessels. Stroke can occur due to several diseases, such as hypertension and unhealthy lifestyles. The tool to establish a stroke diagnosis is ROSIER, or Recognition of Stroke in the Emergency Room, which provides several elements, including clinical symptoms, namely loss of consciousness, seizure activity, asymmetrical face, weakness in the hands, weakness in the legs, experiencing speech disorders, and decreased vision. Method: This study is a case study of patients with acute stroke who came to the ER, patients were assessed using the ROSIER clinical pathway after triage with 7 ROSIER elements when the total score is >0 then the possibility of having a stroke is high but if ≤0 then the possibility of having a stroke is slight. Results: The patient's total score is +2, which means that the patient has had a stroke and is supported by supporting examinations, namely a CT scan of the head, which shows intracerebral hemorrhage. Conclusion: Enforcement using a stroke diagnosis has proven to be effective and fast, so that it can be applied in the ER

Analisis Masalah Spiritual Care pada Pasien

Spiritual Care is an aspect that cannot be separated from the holistic nursing care process. However, until now, spiritual care has not been prioritized, even though spiritual problems have been proven to support the patient's healing process. This condition needs to be followed up, one of which is by continuously analyzing the situation. This research aims to Describe problems that generally occur in the process of Spiritual Care. Researchers used quantitative descriptive research methods. The sample in this research consisted of only 11 samples, with a total sampling technique. Researchers experienced limitations, considering several potential respondents were currently in the educational process. The results of the research show that there are specific variables that hinder the optimization of Spiritual Care, namely the time that nurses have, the patient's awareness, and the lack of training related to Spiritual Care. The author concludes that there is a need for better support with Spiritual Care management. Innovative approaches also need to be given to patients, to increase their awareness.

Pengaruh Terapi Foot Massage dengan Minyak Zaitun terhadap Fatigue Pasien Hemodialisa

Chronic kidney failure is becoming a more pressing problem in global health. One of the interventions of foot massage therapy has been shown to reduce fatigue in hemodialysis patients. Foot massage stimulates sensory nerves in the feet, which activates the parasympathetic nervous system. This causes a decrease in cortisol levels (stress hormones) and an increase in the release of endorphins, neurotransmitters that trigger a sense of comfort and reduce the perception of fatigue. This study used a case study method (case report) by evaluating the implementation of foot massage therapy interventions using olive oil in hemodialysis patients. Evaluation using the FACIT-Fatigue Scale questionnaire which was carried out before and after the action with foot massage therapy, the intervention action was carried out for seven days. The results of this case study after being evaluated using the FACIT-Fatigue Scale questionnaire showed that foot massage therapy for seven days can reduce fatigue. During the intervention period, complaints of fatigue decreased every day. This can be seen from the fatigue score of 14.16 on the first day and 25.23 at the last meeting. It can be concluded that foot massage therapy can reduce fatigue from severe to moderate in chronic kidney failure patients undergoing hemodialysis.

Predictive maintenance is a condition-based maintenance strategy that utilizes machine data to detect potential failures at an early stage before major breakdowns occur. This system integrates automation technology (PLC), the Industrial Internet, and statistical data analysis. This study discusses the application of the Shewhart control chart for automatic monitoring of servo motor torque and pneumatic cylinder cycle time. Torque and cycle time data are read by the PLC and transmitted to a server via Node-RED. On the server, the mean and standard deviation of these parameters are calculated, and the Upper Control Limit (UCL) and Lower Control Limit (LCL) are determined using a 3σ threshold. Data points falling outside the UCL/LCL are identified as process anomalies. In a case study involving several machines equipped with multiple servo motors and pneumatic cylinders, the system successfully detected servo motor torque anomalies before significant damage occurred. The analysis results indicate that with this methodology, the production process can run smoothly, since machine failures can be detected earlier, allowing preventive actions to be properly scheduled outside production hours.
